Coverage for Hospital Stay due to Accidents Benefit
Required documents are:
i. Claim form (Download here)
ii. Certified written authorization for Dai-ichi Life Vietnam to collect information and medical profiles of the victim.
iii. Copy of the ID card of the Policy Owner/Claimant /Payee.
iv. Certified original or copy of the Hospital discharge paper.
v. Copy of the detailed billing documents for inpatient treatment and the Receipt of hospital charges.
