Critical Illness Benefit
Required documents are:
i. Claim form (Download here)
ii. Original Insurance policy.
iii. Certified written authorization for Dai-ichi Life Vietnam to collect information and medical profiles of the Policy Owner/ the Life Insured.
iv. Copy of the ID card of the Life Insured/ Claimant/ Payee.
v. Biopsy results.
vi. Original X-ray, CT scan, MRI, ultrasound films and results.
vii. Certified original or copy of hospital discharge paper.
viii. Certified original or copy of the medical profiles in summary.
