Disability by Accidents Benefit
Required documents are:
i. Claim form (Download here)
ii. Certified written authorization for Dai-ichi Life Vietnam to collect information and medical prifles of the Policy Owner/ Life Insured
iii. Copy of the ID card of the Life Insured/ Claimant/ Payee.
iv. Certified original or copy of the Emergency medicine prescription/ Disability document.
v. X-ray, CT scan, MRI and ultrasound films and results.
