Required documents are:
i. Claim form (Download here)
ii. Original Insurance policy.
iii. Certified written authorization for Dai-ichi Life Vietnam to collect information and medical profiles of the person suffering from the illness or accident.
iv. Copy of the ID card and household registration book of the Claimant/Policy Owner/ Payee.
v. Certified original or copy of the hospital discharge paper and Medical profiles in summary.
vi. Original X-ray, CT scan, MRI and ultrasound films and results.
vii. Original documents related to medical treatment of illness/accident.
viii. Minute on the percentage of work capability loss provided by Board of medical examiners (conducted after 6 months from the date of arising risk).
